It means that Joe was on top of the guard.

It can mean either Joe was physically on top of him, or that Joe began attacking him (even if he was not physically "on" him.) The second one is usually used when someone is using a surprise attack, or taking advantage of a sudden change in situation (like the guard fumbling with his gun.)
